Protecting
the privacy of the personal information that is entrusted to trm is a
responsibility that
we take very seriously. trm is committed to protecting the privacy, confidentiality,
accuracy and
security of the personal information it collects, uses, retains and discloses
in the course of
conducting business.
trm subscribes
to the federal Personal Information Protection and Electronic Documents
Act
(PIPEDA). Our privacy policy contains principles on the collection, protection,
use and disclosure
of personal information and the right of our clients and customers to
have access to personal
information about themselves and, if necessary, to have the information
corrected.
Privacy
Principles
trm incorporates the following ten principles into its dealings with private
information provided
to us by our clients, customers, colleagues, and business partners. These
principles address the
way in which trm collects, uses, and discloses personal information. It
also addresses the rights
of individuals to have access to their personal information and to have
it corrected if necessary.
The code's
10 principles are:
1. Accountability: trm is responsible for personal information
under its control and has designated
James MacDonald as its Privacy Officer. James MacDonald is accountable
for the organization's
compliance with the following principles.
2. Identifying Purposes: The purposes for which personal
information is to be collected is identified
by trm at or before the time the information is collected.
3. Consent: The knowledge and consent of the individual
is required for the collection, use or
disclosure of personal information, except when inappropriate.
4. Limiting Collection: The collection of personal information
is to be limited to that which is
necessary for the purposes identified by trm. Information shall be collected
by fair and lawful means.
5. Limiting Use, Disclosure, and Retention: Personal
information shall not be used or disclosed
for purposes other than those for which it was collected, except with
the consent of the individual or
as required by the law. Personal information is retained only as long
as necessary for fulfillment of
those purposes, or as legally required.
6. Accuracy: Personal information is to be as accurate,
complete, and up-to-date as is necessary for
the purposes for which it is to be used.
7. Safeguards: Personal information shall be protected
by security safeguards appropriate to
the sensitivity of the information.
8. Openness: trm will readily make available specific
information to individuals about its policies
and practices relating to the management of personal information.
9. Individual Access: Upon request, an individual shall
be informed of the existence, use
and disclosure of his or her personal information and shall be given access
to that information.
An individual shall be able to challenge the accuracy and completeness
of the information
and have it amended as appropriate.
10. Challenging Compliance: An individual shall be able
to address a challenge concerning
compliance with the above principles to our privacy officer, James
MacDonald,
at jmacdonald@totalrehab.ca., our designated
privacy officer.
Privacy
with respect to our website
trm does not collect any unique information about visitors to our website
(such as your name,
e-mail address, etc.) except when visitors knowingly provide such information.
trm only collects
your personal data to the extent that they are necessary to provide you
with our services, for
the development of web statistics and to comply with mandatory requirements.
trm collects
and stores some visitor information regarding the usage of this website,
such as date and time
of day you access our website, browser type, browser language, the Internet
Protocol (IP)
address of the computer you are using, number of hits, the pages visited,
previous/subsequent
sites visited and length of user session.
trm will
do its best to ensure that the processing of your personal data is carried
out in such
a way that your privacy is protected and safeguarded as much as possible.
The information
mentioned above is gathered by using a cookie. A cookie is a small line
of
text that is stored by your browser on your computer's hard drive. Our
